The parent(s) and/or the school contact Child Find to request an evaluation
The Child Find Liaison to the private school sets up a referral meeting at the school and the school personnel and the parent(s) are invited to attend. Initial data is collected at the referral meeting. Parent signs Request for Evaluation of Permission form.
If a Specific Learning Disability is suspected, the school can collect the Progress Monitoring Data the six weeks prior to requesting the evaluation/scheduling the referrral meeting where the Permission for Evaluation is signed, or after the permission is signed. Please note that all the data has to be collected within 60 days of when the permission is signed. The date that the Permission to Evaluate is signed sets the 60-day compliance date - the date in which a meeting needs to be held to examine testing results and the data collection in order to determine Eligibilty for Special Education Services. If six Progress Monitoring Points have not been collected within the 60 days, the meeting will have to be held and the determination would be that the child does not qualify for a Specific Learning Disability because there had not been enough data collected by the school.
At the eligibility meeting, Determination of Eligibilty is completed and a Prior Written Notice is presented to the parent(s) regarding Proposal or Refusal of Services.
If a student attending a private school qualifies for an Individual Service Plan ( qualifies for a Specific Learning Disabilty in Reading, Kindergarten through 3rd grade), then an Individual Service Plan is coordinated with the DPS ISP Coordinator, the School Personnel and the Independent Agency Contracted by Denver Public Schools to provide the direct services
Students who qualify for an Individual Service Plan are monitored on an annual basis.
Students who qualify for special education services (ISP or not) are re-evaluated every three years, in order to determine continued eligibilty and/or continuation of ISP services.
